  • Medical data monitoring services: Eligible individuals can receive two years of CyEx Medical Shield Complete coverage, which includes $1 million in identity theft insurance as well as monitoring for medical data exposure.
  • Out-of-pocket documented losses: Claims of up to $3,000 can be submitted for losses sustained between October 17, 2024, and February 5, 2026. This includes reimbursement for identity theft or fraud, credit monitoring fees, replacement of IDs, and postage costs for contacting financial institutions.
  • Lost time reimbursement: A maximum of $100 can be claimed for up to four hours of time spent addressing issues caused by the breach, such as changing passwords, investigating suspicious activity, or researching the incident. The time is reimbursed at a rate of $25 per hour.

How to Submit a Claim

Class members must file their claims either online or by submitting a completed PDF claim form via mail or email to the settlement administrator. Supporting documentation, such as bank or credit card statements and receipts, is required for claims related to out-of-pocket losses.

The deadline to file a claim is February 5, 2026. Payouts will be distributed approximately 60 days after the court grants final approval of the settlement, scheduled for March 6, 2026. Claimants can choose to receive their payments through PayPal, Venmo, Zelle, a virtual prepaid card, or a paper check.

Breakdown of Settlement Fund

The settlement fund covers various costs, including:

  • Administrative expenses
  • Attorneys' fees, capped at $200,000
  • A $2,500 service award to the class representative
  • Payments for medical monitoring services
  • Reimbursements to approved claimants

The total amount allocated for individual claims will depend on the number of valid submissions.

  • Claim submission deadline: February 5, 2026
  • Opt-out deadline: February 19, 2026
  • Final approval hearing: March 6, 2026
